Abstract: 

Background and Objectives: Blood transfusion related alloimmunization is common among patients receiving regular blood transfusion, especially thalassemia patients. In this study we aimed to find the prevalence of alloimmunization among patients of thalassemia. Materials and Methods: In this desciptional study, we screened 305 thalassemia patients (53. 8% female and 46. 2% male, age 4-38) referring to Imam Khomeini Hospital during 2017-2018 for presence of antibodies against blood group antigens. Then for those patients of positive antibody screening result we identified the antibody in each patient. Results: Seven out of 305 patients in this study had positive antibody screening results. The most prevalent alloantibodies were detected to be anti-K (6 patients, 85. 7%) and anti-E (2 patients, 28. 57%). Also, 2 out of 7 patients with positive antibody results presented dual antibody of anti-K and anti-E. Conclusions: According to the results, anti-K and anti-E are the most likely antibodies presenting in patients of thalassemia with positive antibody screening results. This point is important to bear in mind in providing special blood supplies for thalassemics who are in need of regular blood transfusion. Prior to treatment of multitransfused patients like thalassemia the phenotyping of patients for important blood groups of Kell and Rh should be considered.

Abstract: 

Background: Postpartum bilateral femoral neck fracture (BFNF) is a rare condition. We here report a case of BFNF due to excessive corticosteroid consumption, twin pregnancy, immobility, and vitamin D deficiency.Case: This is a report of a 32-yr-old woman with bilateral femoral insufficiency fracture five days after emergency cesarean section due to preterm labor, twin pregnancy, and the history of a previous cesarean section at 33 wk. Antenatal repeated courses of betamethasone injections for fetal lung maturity, daily oral use of prednisolone for the history of miscarriage, immobilization, and vitamin D deficiency were the important contributing factors in her past medical history and lab investigations. The bone mineral density examination showed low bone density for the expected age.Conclusion: Clinicians, who deal with pregnant women, should consider the diagnosis of bilateral femoral insufficiency fracture in any pregnant women with pelvic pain. Awareness of risk factors of BFNF might help to reduce the rate of this complication.

Abstract: 

Background and Objectives: There are many antibodies and antigens that cause blood group related transfusion reactions. In this study we aim to find the prevalence of blood group antibodies in patients at different ages and with different gender. Materials and Methods: In this descriptive study, we screened 30659 patients (59% female and 41% male, age range 1-85) referring to different wards of Imam Khomeini complex hospital during 2017-2018 for presence of any antibodies against blood group antigens. Then, for those patients with screened positive antibody we identified the antibody in each patient. Finally we categorized the antibody positive patients based on their main ABO and Rh blood group of all wards such as cancer, hematology, general and also separately for postpartum ward. Results: Out of 30659 patients in all wards, 123 and out of 4364 just in postpartum ward, 20 patients had positive antibody. The most prevalent alloantibodies were detected to be anti-E(31. 7%), anti-K(30. 8%), and anti-D(14. 6%) in order. Conclusions: Anti-E and anti-K are the most likely antibodies presenting in patients with positive antibody screening results awareness of which can be helpful in storing blood components with rare blood groups. Moreover, the evaluation of common alloantibodies in the postpartum ward shows that the administration of RhoGAM might not be adequate in HDFN prevention and other antibodies should be considered in this regard.

Abstract: 

In cold season, draught or UNDESIRED local cooling sensation in ankle and neck region is one of the most frequent cases of complaint of the occupants. A person who are subjected to draughts in winter, tend to elevate the room temperature to counteract the cooling sensation, thereby increasing the energy consumption. In naturally ventilated buildings, draught is due to windows and other cold surfaces in the room. Draught is dependent on the air speed and on the magnitude of turbulence intensity. Serious draught complaints can often occur at mean speeds lower than those recommended by standards when turbulence intensity is high. So investigation of UNDESIRED local cooling in floor heating systems is very important, although in these systems the mean air speed is not significant. In this paper, the effects of size of window on draught are investigated in floor heating systems. Results demonstrated that, UNDESIRED thermal discomfort caused by local cooling phenomenon in floor heating systems is negligible. At the end, the probability of occurrence of local cooling phenomenon in floor heating systems is compared to vertical heating panels. Thereby the floor heating systems are more effective than the vertical heating panels in aspect of thermal comfort and energy consumption.

Abstract: 

This study was conducted to investigate the overall efficiency of production and transmission of electricity. For this purpose, sixteen areas of production and transmission of electricity industry of country were selected as decision-making units. Based on Khalili and Shahmiri model, one mathematical model was proposed that includes two stages of the production and transmission of electricity. In the proposed model at the end stage of the production, output variables were considered which do not enter to next stage of production. In the second phase, in addition to inputs received from production stage, other input variables were considered that are not included to model from previous stage. In the first stage, variables of domestic consumption of power stations and consumed fuel as input variables maximum production load variables, special production, gross production, efficiency, nominal power of power station, and practical power of power station were considered as outpour variables. In the second stage, variables of maximum production load, especial production, gross production, capacity of power transmission posts and transmission lines were considered as input variables, delivered energy variable was considered as desired output, and energy losses were considered as UNDESIRED output variable. Results of two stages indicated that districts of Azerbaijan, Isfahan, Tehran, Khorasan, Khuzestan, Semnan, Kerman, and Fars were efficient.

Abstract: 

THIS PAPER DEALS WITH THE PROBLEMS OF DETECTING A CHIRP SIGNAL OF MOTION TARGET AGAINST A STRONG UNDESIRED SIGNAL FROM THE TRANSMITTER IN RADAR. WE USE GENERALIZED ALMOST-CYCLOSTATIONARY (GACS) SIGNAL PROCESSING METHOD TO DETERMINE THE RATE AND INITIAL FREQUENCY OF THE CHIRP SIGNAL IN PRESENCE OF THE STRONG UNDESIRED SIGNAL. THIS TECHNIQUE WHICH EXPLOITS THE SECOND ORDER CYCLOSTATIONARY TO DETECT FEATURES OF THE CHIRP SIGNAL IN LOW SNR HAS DESIRABLE PROPERTIES. THE STUDY UTILIZED A METHOD TO DETERMINE AND CHARACTERIZE SIGNALS FOR HIGHLY ADVERSE SIGNAL TO INTERFERENCE PLUS NOISE AND EXPLAIN HOW USING CYCLOSTATIONARY PROPERTIES CAN EXTRACT FEATURES OF CHIRP SIGNAL FROM THE MIXTURE RECEIVED SIGNAL. THE THEORY AND SIMULATION OF THIS METHOD INDICATED THAT THE CHIRP RATE DEPENDS ON THE LAG PARAMETER AND THE CYCLE FREQUENCY.

Abstract: 

Objective: Isolated salpingeal torsion (IST) is a rare cause of acute abdomen in women of the reproductive age group. The lack of pathognomonic clinical, laboratory or radiographic findings makes early diagnosis a challenge. We describe a case of IST in a 13-year-old who suffered from a repeat torsion following initial conservative management. Case Report: A 13-year-old girl presented with acute right lower quadrant abdominal pain of insidious onset. Her past medical history was non-contributory and her initial workup was unremarkable. Exploratory laparoscopy subsequently revealed isolated torsion of the right fallopian tube which was detorted. She presented 10 months later with similar complaints and further workup demonstrated right adnexal torsion which was confirmed by laparoscopy,salpingo-oophorectomy was necessary due to gangrenous necrosis. Conclusion: Conservative management of fallopian tube torsion confers the advantage of fertility preservation but the risk of repeat torsion remains clinically significant.

Abstract: 

Vibration control of large flexible structures in new dynamic systems has significantly encountered with many challenges. In this area, there are several approaches of vibration control implemented on complicated dynamic systems in which the change of vibrational characteristics with the time of process leads to performance violation. In this paper, regarding the estimation of UNDESIRED vibration frequency of a flexible structure, input control of the dynamic system is reconstructed. In this regards, the input of the dynamic system is made to minimize the magnitude of the vibration. This strategy in which the control input is constructed by means of UNDESIRED vibration frequencies is similar to use of anti-viruses in medicinal approaches. The proposed control strategy is implemented on yaw channel of a flexible sounding r cket in order to reduce the destructive effects of bending vibration. The system responses show the effects of the vibration on the yaw channel of the system are significantly decreased.

Abstract: 

In this study, considering slippage between a robot end-effector and an object, adaptive control of a one-finger hand manipulating an object is explored. This system is a good sample to develop different techniques such as grasp analysis, grasp synthesis, stability analysis and designing different types of controller for cooperative manipulator systems. Due to the presence of inequality equations in frictional point contact modeling, a novel formulation is developed to replace the equality and inequality equations with a single second order differential equation with switching coefficients. Introducing this new friction contact model, an input-output conventional form is derived using the equality and inequality equations of motion of the system. Using this new form of motion equations, two adaptive controllers with simple update laws are proposed that both of them ensure the asymptotic convergence of the object position tracking as well as slippage control while compensating the system uncertainties. The first controller compensates the uncertain masses of the manipulator links and the object while the second one compensates the uncertain coefficients of friction. Numerical simulation is utilized to evaluate performance of the proposed controllers.
